Program Coordinator

Summary of Primary Duties and Responsibilities:

  • Oversee day-to-day operation of services in assigned areas.
  • Act as a mentor and support to the positions supervised, and conduct performance reviews and evaluations.
  • Provide training, development, and support to all staff to ensure quality of services and successful outcomes for people served through programs.
  • Assist with development of program curriculum for both individuals and groups in accordance with ISP goals, provide opportunities for inclusion and is responsible for effective utilization of consumer budgets and staff caseload assignments.
  • Maintain confidentiality and ensure efforts are undertaken to implement each participant’s choices records of services provided.
  • Work in conjunction with Director and Service Coordinator to ensure agency success and uphold and promote agency’s mission statement.
  • Coordinate and implement all required reporting, maintain records and provide a clean, safe working environment.

 

Qualifications/Requirements:

  • High School Diploma or GED required.
  • Minimum of 3 years experience working with persons with disabilities required.
  • Minimum of one year of supervisory experience
  • Demonstrated ability to teach and supervise others.
  • Knowledgeable in the development of Individual Service Plans. Must demonstrate sound understanding of Community Integration, and the ability to work effectively within human service systems
  • Excellent verbal and written communication skills.
  • Work non-standard/flexible schedule
  • Valid driver’s license and dependable transportation.
  • Ability to lift a minimum of 35 pounds; upper body leverage strength required.

Employment Consultant

The candidate will develop employment opportunities and coordinates with employers the hiring, training, and necessary on-going supports required by individuals with disabilities to maintain employment. Facilitate positive relationships between co-workers with and without disabilities to ensure successful employment and to maximize opportunities for lasting relationships. Assist and support people with disabilities in gaining citizenship, involvement in their communities, and the development of friends and supports outside of the human service system.

Team Lead

The candidate will assist persons with disabilities in pursuing their personal futures and goals through residential supports and community integration. Assist with the design, implementation and monitoring of support plans to ensure meaningfulness. Act as a mentor and support to ensure successful outcomes for people served through programs. Maintain case records and documentation for persons/locations assigned. Uphold and promote the agency’s mission statement.

Direct Support Professionals (DSP) / Community Consultant

Direct Support Professionals assist individuals with disabilities live self-directed lives, realizing dreams and enjoying the daily liberties and human rights others take for granted. DSP’s assist consumers in developing daily living, self-help and social skills through individual-specific supports and community integration opportunities. Responsibilities vary based on the work setting.  Community Consultants provide direct supports and flexible assistance in community and residential settings.

Job Qualifications

  • High school diploma or demonstrated equivalent competency required.
  • 18 Years of age or older.
  • Experience working with persons with disabilities in the community preferred.
  • Excellent people and communication skills required.
  • Ability to work flexible/non-standard hours.
  • Ability to lift a minimum of 35 pounds; upper body leverage strength required.
  • Valid driver’s license and dependable transportation.
  • Ability to keep accurate documentation.
  • Knowledge of community and community activities preferred.

Therapists serve as members of multi-disciplinary teams, assisting in developing and implementing the Individual Family Support Plan outcomes for the children and families served. Performs discipline-specific therapy and evaluations for infants and toddlers referred to the First Steps program.

Behavioral Specialist

Works in collaboration with other members of the treatment team, designs and directs the implementation of a behavior intervention plan which is individualized to each individual as well as to the family needs and consumer’s daily activities. The Behavioral Specialist identifies behavioral goals and intervention techniques and recommends behavioral change methods. Provides staff training and designs data collection formats. Serves as a resource to staff, parents and providers. Attends quarterly and annual meetings as related to consumers needs.

Job Qualifications

  • Master’s Degree in: Clinical Psychology, school psychology or another applied health service area of
    psychology; Special Education; Social Work or Counseling
  • Licensed Marriage and Family Therapist; licensed clinical social worker or licensed mental health
    counselor
  • Knowledge and ability to demonstrate coursework in or 5 years of experience in devising, implementing
    And monitoring behavior support plans.
  • Demonstration of excellent oral and written skills.

Recreational Therapist

Works in collaboration with other members of the IST, designs and implements a recreational intervention plan which is tailored to each individual and his/her daily activities. Utilizing the APIE standards, the Therapist will consider input from other IST members as well as the individual served, conduct a standardized assessment and develop a therapeutic program targeted towards each individual’s needs and identified goals. The Therapist will provide treatment services and recreation activities for individuals using a variety of techniques in an effort to improve and maintain the physical, mental, and emotional well-being of their clients so that they can enjoy greater independence. Techniques can include arts and crafts, sports, games, dance and movement, drama, music, and community outings. The Therapist will document the consumer’s progress during activities. Therapists will uphold and promote the agency’s mission statement.

Must possess the following:

  • Bachelor’s degree or higher with a major in Therapeutic Recreation or a major in recreation or leisure with an option in Therapeutic Recreation; or Bachelor’s degree or higher in any major with a minimum of five years full-time paid work experience in Therapeutic Recreation services; or Bachelor’s degree or higher in any major with a minimum of one year of full-time paid work experience in Therapeutic Recreation services under the supervision of a Certified Therapeutic Recreation Specialist (CTRS).
  • Have a Certified Therapeutic Recreation Specialist (CTRS) credential by the National Council for Therapeutic Recreation Certification (NCTRC)
  • Minimum of 1 years experience working with persons with disabilities required.
  • Demonstrated ability to teach others.
  • Knowledgeable in the development of Individual Service Plans. Must demonstrate sound understanding of Community Integration, and the ability to work effectively within human service systems

Van Driver

Transport individuals as assigned, providing assistance as necessary. At all times, ensure the safe operation of the vehicle, including safe boarding and unloading of passengers. Drivers must have the ability to complete daily pre-trip inspection and accurately complete service logs. Good driving record, valid driver’s license and ability to obtain a public passenger chauffeur’s license required.
