Residential Support - Supported Living

Residential supports are provided to assist individuals in building skills needed to live in a safe environment.  In-home support may be provided 24/7, or on a scheduled basis where assistance is provided for a planned number of hours on the same days/same times each week.  Persons receiving residential services work with their support teams to define individual goals, then service hours are scheduled for activities to obtain those goals.  Support teams include Sycamore residential assistants as well as a Case Manager or Service Coordinator and other appropriate service provider representatives.

In-home Supports may include:

  • Budget and financial assistance
  • Learning to make choices with effective outcomes
  • Coordination of health care
  • Instruction in planning and preparation of nutritious meals
  • Making connections with community resources
  • Basic home care and maintenance
  • Assistance with personal care needs

Community based activities are combined with in-home supports to improve skills that will lead to a more independent, involved life for the person receiving services.

In-community supports may include:

  • Making connections that build friendships
  • Joining clubs or other organized group activities
  • Becoming a volunteer at a community organization
  • Participating in communal social gatherings
  • Attending events

Someone is always available with our 24/7 on-call system.
